Paralegal/Medical Records Analyst

Hassan, Hassan & Tuchman, P.A
08.2017 - 11.2017
  • I analyzed medical records from various hospitals and care facilities, to get diagnoses for clients. I sent out HIPPA request out to various medical offices, hospitals, physical therapy centers, surgical centers, long-term care centers, etc. to obtain medical records that would be used to create medical summaries to send off to the insurance companies. I called various hospital, physicians, radiological, surgical, and anesthesia billing offices to get information on how much of the client’s bill was paid, and by whom (be them Medicaid, Medicare, personal injury protection (PIP), or medical insurance). I answered phone calls from clients inquiring about the status of their cases and kept a log of when they called and what they asked about. I kept a log detain what I did on a specific case on a specific day. I handled various cases in a high-volume law firm, at one time I had around ninety files in my possession. I kept a excel spreadsheet to keep track of all my cases and made sure that they did not fall between the cracks. I used medical diagnoses and reports to create detailed medical summaries for the insurance companies to review. I assisted with client intake, where I would ask a series of questions to figure out what type of injury was incurred, as well as any information we would need to further pursue their case (e.g. Name of Employer, Name of Spouse, Address, Employment Title, Salary, Injuries sustained, A detail about what happened during the accident, Insurance carrier for auto, medical insurance, personal injury protection, who was driving the car, who owned the car, who was the driver that caused the injury, what was their care make, how fast were the driver going, how fast were you (or the driver) going, etc.) in order to make sure there is a case worth pursuing. Assisted with the litigation side of the team, in collecting medical records or outstanding reports needed for them. Utilized a fax machine to make copies, and to fax requests out to various medical offices, hospitals, surgery centers, etc. Used Microsoft Office for day-to-day operations. Mailed out requests using a letter head to various medical offices, surgery centers, physical therapy centers, etc. to obtain billing records or medical records for the client’s case.
